TASC 90: Population Health
Learn the key features of the Population Health Portal, including a readiness assessment, resources and data scenarios to support an organization’s journey into population health. Learning objectives include: Discover how a state Flex Program can leverage the resources, assessment and data scenarios in the Population Health Portal to support their critical access hospitals (CAHs) and develop relevant Flex Program activities based on CAHs’ needs.
