On September 2, 2010, Deputy Jeremy Banks of the St. Johns Sheriff's Office in Florida found his girlfriend, Michelle O'Connell, dead from a gunshot wound. He was allegedly standing outside the room when she shot herself with his duty firearm. Ignoring the obvious conflicts of interest, Sheriff David Shoar handled the case internally, which potentially compromised the entire investigation.
